SAI Redesign Development Group

 

The Department of Health are progressing a programme of work to redesign the Serious Adverse Incident (SAI) Procedure, seeking to address relevant recommendations arising from the Inquiry into Hyponatraemia-related Deaths (IHRD) and the Independent Neurology Inquiry (INI) alongside recommendations from the RQIA Review of Systems and Processes for Learning from SAIs. 

As part of this redesign programme the Department will recruit two service user, carer or family representatives to sit on the SAI Redesign Development Group. The individuals will be part of the team, together with policy and professional leads from across the health system who will be responsible for developing the new framework for the identification, review and capturing of learning from patient safety events.

Applications will be invited from individuals who have had experience of having been involved in a Serious Adverse Incident review in the last 5 years, and/or a general interest in patient safety issues and improvement of Health and Social Care services.
