Patient and Client Council in the community
Understanding that many people face challenges when accessing services and participating to inform decision–makers of the access issues they face, this approach will provide;
- trained staff at physical locations, to listen to individual’s concerns/ issues about health or social care;
- free support and advice, to help individuals find a resolution or raise concerns appropriately;
- access to resources tailored to their needs;
- support through a complaints process, which will be always be focused on ensuring the individual’s voice is heard and listened to.
“Our vision is for a Health and Social Care service actively shaped by the needs and experience of the public and we recognise that health inequalities are a significant barrier for many in our communities” said Meadhbha Monaghan, PCC Chief Executive. “By embracing partnership working, and developing our ‘network of networks’ approach, with organisations already embedded within local communities, PCC Support and Connect can provide a flexible and responsive service for the public to ensure that as many people as possible have the opportunity to access our services and have their voice heard. By prioritising inclusivity and support, this initiative aims to prevent people falling through gaps in services and promote better health outcomes.”
